Fonemed offers the first Disease Management Program based on clinical protocols and best practice guidelines specifically designed for remote use in managing diabetes, COPD, asthma, CHF and behavioral health. Our Disease Management Program, developed with support from the government of Canada, draws on the U.S. Coleman, Wagner and Veterans Administration models, as well as the Whole Health Program of Great Britain and more than 17 years of our own experience providing remote care in the telehealth industry. Using telehealth as a key element in the continuum of care, we improve the patient experience, provide expanded touch points and significantly reduce costs. We support newly-diagnosed patients as well as patients at every stage of their diseases. This includes both short-term programs to reduce hospital readmissions and longer term disease management.
Our core Disease Management Program involves frequent interaction with patients and the continuous availability of health counseling costs that are about half of the price that of a single visit by a nurse. In similar comprehensive approaches over a three-year period, the Whole System Demonstrator Programme of the British Department of Health showed that if used correctly, telehealth can deliver a 20% reduction in emergency admissions, a 14% reduction in elective admissions and a 14% reduction in bed days. More strikingly, they also demonstrate a 45% reduction in mortality rates. In more than 2 million patient encounters, Fonemed has consistently shown an ROI of more than 200% for our remote nurse advice programs and saved our clients more than $30 million in 2013.
